Again, I’d propose coordinated service navigation and leveraging of public-private partnerships to improve service like transportation to primary care appointments, home delivery of meals or other resources, healthcare service navigation (including assistance in obtaining and tracking appointments with specialists, obtaining and tracking results from those specialists and coordination with primary care providers, advocacy and language support,  transportation to rec centers that could potentially include first-tier case management and support to help catch challenges that–otherwise unchecked–might result in costly and intolerable health outcomes, costly healthcare visits, and negative quality of life experiences.
